All volunteers who work with children, youth, or vulnerable adults at a parish are required by the Diocese of Rochester to complete CASE Training, authorize a criminal background check, and sign the volunteer code of conduct.
CYO Coaches have additional training requirements. This additional training is not part of C.A.S.E. though it shares the same training platform.
To complete requirements for CASE, volunteers will need to bring to the Faith Formation Office (which handles CASE for the parish):
The certificate that shows you have completed the training.
The filled out, signed, background check form (two-sided)
The signed receipt of Code of Conduct. (You retain the actual Code of Conduct. Submit the signed "receipt" document which states you have received and agree to the code of conduct).
A photo id (such as a valid driver’s license) to verify date of birth, plus your social security number. If you have a current: “Enhanced” Driver’s License, Real ID, or Passport, you may choose to submit a photo copy of one of those forms of identification instead of providing your social security number.
Headshot (does not need to be professional) taken within the last 3 months emailed to the Faith Formation Office.
Current C.A.S.E. Forms and on-line training instructions can be found in the 'Documents' section of this page. Please discard out of date forms.
